Expert support in your health and wellbeing goals, in conjunction with your healthcare team to help build your ability to maintain a healthy lifestyle.
Minor home modifications
Minor home modifications to aid in a safe and independent lifestyle; fitting of grab/handrails, bed/chair raisers, installing ramps or non-slip treatments.
Transport
Help to access door-to-door transportation services for those unable to use alternate modes of transport due to health or mobility reasons.
